Life Care Consultants has been created for the purpose of providing coaching and consulting for families as they face decisions regarding the care of loved ones. It is critically important to know what resources are available and to have someone to provide knowledgeable assistance when a life-changing event requires difficult decisions. LCC will coach clients through these complex decisions and provide a series of services that are specifically designed for each client's needs.
Menu of Services
I. Preservation of Client Assets
A. Identification of Assets (e.g., real estate, investments, pensions, life insurance, etc.)
B. Consultation with a Financial Advisor to develop and analyze strategies and development of an asset management plan.
C. Quarterly Statement of Asset Performance.
D. Quarterly/Semi-annual review with client.
II. Fiduciary Services
A. Bill Pay Services -- automatic timely payment of financial obligations.
B. Income Management -- ensures that money provided for the welfare of an individual is spent on priority needs and expenses.
C. LCC Staff may assume one of the following roles:
1. Trustee -- an individual or entity who holds or manages assets for the benefit of another.
2. Personal Representative/Power of Attorney -- legal authority to act on someone's behalf regarding a wide range of medical/financial decisions.
III. Comprehensive Care Consulting
A. Coordination of medical and psychosocial evaluations.
B. Development of an Individualized Service Plan.
C. Assessment of Environmental Needs.
D. Application for Public Benefits (if appropriate) and participation in appeals process (if necessary).
E. Monitoring and coordination of medical care.
F. Identification and linkage with necessary community-based services.
G. Consultation with principal parties involved with client's care (family members, health professionals, attorneys, etc.).
H. Maintenance and updating important client data (e.g., medical, legal, financial records).
IV. Advocacy
A. Professional representation regarding community resources and public benefits.
B. Negotiation and mediation, if necessary, for conflicts arising over the care of an elderly family member.
C. Advocacy action aimed at improving the care and supervision of a family member.
D. Follow up to see that corrections and solutions occur.
E. Providing a forum for supportive action on behalf of clients and their families.
F. Providing appropriate referrals for a broad array of medical services and community resources.
G. Participation in mediation for conflict resolution with or on behalf of clients and their family members.
H. Complaint resolution and resident rights representation for nursing and personal care homes.
I. Assisting in the protection of clients from health care fraud and abuse.
